By completing and submitting this form, I voluntarily authorize D-Tech Staffing ("Company") to deposit my compensation directly into my designated bank account(s) via electronic funds transfer (EFT). I also authorize my financial institution(s) to accept and process these deposits on my behalf.

In the event of an erroneous deposit, I authorize D-Tech Staffing to initiate a correcting debit transaction to withdraw the incorrect amount, provided the withdrawal does not exceed the original deposit. If the erroneous deposit has already been withdrawn or is otherwise unavailable, I agree to cooperate with D-Tech Staffing to resolve the issue.

I understand that I may revoke this authorization at any time by providing written notice to D-Tech Staffing. Revocation will take effect only after the Company and my financial institution have had a reasonable period to process my request. I also acknowledge that I may request payment via alternative methods if needed.

This authorization will remain in effect until I submit a written request to terminate it, allowing D-Tech Staffing and my financial institution a reasonable processing period.

I confirm that the banking information I provide is accurate and complete, and I agree to notify D-Tech Staffing immediately of any changes to my bank account details. I understand that D-Tech Staffing is not responsible for delays or errors in deposit processing due to incorrect or outdated information provided by me.
